Families Coming with the platform

The family is part of the care.

A daughter in another city wants to know that a nurse came today. Whoever is with the patient at home needs to know who is at the door, and when. Today they call the office and ask. ProfMed is building the Family Window so they can simply look.

No medical notes. No diagnosis. No medicines. No bills.

The patient decides who may look, and can stop it at any time.

Nothing to download.

Why this matters

In home health, hospice and every other outpatient service, the office spends part of every day answering the same questions on the phone. The family is not asking for medical records. They are asking the simplest human questions.

Every one of those calls takes a staff member away from care. And the person who most needs the answer – whoever is sitting with the patient – is usually the last to get it.

What the Family Window will show

  1. The office invites the personYour staff adds the family member: the name, the relation and the cell phone number.
  2. The patient says yesThe patient, or the person who signs for the patient, approves. The approval is recorded with the date.
  3. They get a linkOne link by message. No app store, no download, no password to remember. Every time they open it, a 6-digit code is sent to their phone.
  4. They see the visitsThe next visit with the day, the time, who is coming and their profession. The visits this week. And the visits already done, with the time each one finished.
  5. And nothing elseOnly this one patient. Only the visits. Nothing else in the system is reachable from that link.

The person who opens the door

Whoever is with the patient at home – a daughter, a son, a spouse, a neighbour – needs to know who is coming and when.

  • That the professional is on the way, and about when to expect them.
  • What to have ready before the visit.
  • Two buttons to the office: “Nobody will be home” and “Please call me”.

What it will never show

This is the part we designed first, not last.

  • No visit notes, no assessments, no diagnosis.
  • No medicine list and no medication record.
  • No bills, no insurance, no payment information.
  • Nobody can change anything. They can only look.

Why the family can trust what they see

The Family Window is not a message somebody typed. It is the record itself.

  1. Every visit is recorded where it happensThe professional checks in at the patient’s home on her phone. The time, the date and the place are written down at that moment – not from memory at the end of the day.
  2. Signed by two peopleA visit is finished only when the patient or the family member signs, and the professional signs. Two signatures, or it is not done.
  3. It cannot be changed afterwardsOnce a visit is closed, the record stands. Nobody can go back and make it look different.
  4. Nothing is ever deletedA visit that was cancelled stays cancelled in the history. A visit nobody attended stays on the record. The family sees the truth, not a tidy version of it.

So when the family opens the link and sees “nurse visit finished 11:14 AM”, that is the real moment, from the real place, signed by two people.

The patient stays in control

Access is given by the patient, not by us and not by the agency.

  • The patient, or the person who legally signs for the patient, approves each person by name.
  • Any person can be turned off at any time. When they are off, the link stops working the same minute.
  • Every approval and every removal is recorded with the date.
  • The link alone is not enough – a 6-digit code is sent to the phone each time.

Where this stands Coming with the platform

Honest status

The Family Window is designed and is being built as part of the ProfMed platform. It is not available to families today. We are saying so plainly here rather than leaving it unclear: what you have read on this page describes what is coming, not what is live.

What is live today is the platform underneath it – agencies sending jobs, professionals accepting and signing, and every visit recorded with the time, the date and the location. The Family Window shows the family a small, safe part of that same record.

If you are an agency, a funder or a partner and you would like to see the design, ask us for a demo.
